Required Paperwork and Orientation

Before you begin your rotation, please ensure that you have completed your general application. If you have not, you will need to complete it here.

You will need to complete our Preceptorship Packet and Acceptable Use Policy (They are fill & sign documents). You are also required to submit your drivers license, background check, and vaccination records. The following vaccinations are needed.

• TB Test: negative results within 12 months. If student has a positive skin test or has ever had one, they will need to show proof of a normal chest x-ray or a T-Spot blood test.

• Tdap Vaccine within the last 10 years

• MMR Vaccine (2)

• Bacterial Meningitis within past 5 years of 1st day of class (under age 22)

After your paperwork is approved, you will need to schedule your orientation with the Program Coordinator. Orientation will occur at your host clinic, 15 minutes prior to the beginning of your first shift. Please schedule your first hours with your preceptor prior to scheduling orientation.
